Neue Leitlinie gastroösophageale Refluxkrankheit 2022 – Paradigmenwechsel in Diagnostik und Therapie
Zusammenfassung: Die gastroösophageale Refluxkrankheit (GERD) ist häufig. Die Diagnose kann im Einzelfall schwierig sein, da kein diagnostischer Goldstandard existiert. Aus diesem Grund wird zwischen Refluxbeschwerden mit und ohne gesicherte Refluxkrankheit unterschieden. Endoskopie mit Histologie, Impedanz-pH-Metrie und hochauflösende Manometrie kommen situationsabhängig in der Abklärung zum Einsatz und gelten in dieser Kombination als der diagnostische Goldstandard. Allgemeinmaßnahmen wird im Management der Refluxbeschwerden ein hoher Stellenwert zugemessen. Protonenpumpeninhibitoren (PPI) gelten weiterhin als Mittel der ersten Wahl für alle klinischen Manifestationen. Bei genügender Symptomkontrolle aus Patientensicht sind andere Pharmaka aber ebenso akzeptabel. Ausgenommen von dieser Empfehlung sind Patienten mit schwerer Refluxösophagitis (Los Angeles Grad C/D und peptischer Striktur), die a priori eine PPI-Dauertherapie erhalten sollten. Persistierende Refluxbeschwerden unter PPI-Therapie sind häufig. Im ersten Schritt muss die Ursache für das Therapieversagen geklärt werden. Bei PPI-refraktärer Refluxkrankheit kommen pathophysiologisch orientierte Kombinationsbehandlungen und Antireflux-Interventionen zum Einsatz. Eine Antireflux-Operation ist nach entsprechender Abklärung eine Option für Patienten mit schwerer Refluxösophagitis, peptischer Striktur, unzureichendem PPI-Effekt infolge ungenügender Refluxkontrolle und PPI-sensitiver GERD, die eine Dauermedikation nicht wünschen oder die Medikation nicht vertragen.
